Formerly the Centerville Good Samaritan Center, Centerville Care and Rehab has been serving the community of Centerville and surrounding areas since first opening doors in 1961. In the past 50 years, we have kept our home town, family atmostphere while continuing to improve our quality of care by taking advantage of advancements in rehabilitation services, medical technology and employee education.

Sam Van Voorst, Administrator
Sam recently graduated from USD obtaining his Health Care Administration degree. He currently resides in Vermillion, with plans to move to Centerville with his Fiancee Marysa! Sam and Marysa plan to get married June of 2019.
Patricia Knutson, Director of Nursing
Patty has been employed at the facility since September 9, 1997. At initial employment, she worked as a day shift charge nurse until 2007. At that time, she became the MDS Coordinator and monitored both inpatient and outpatient scheduling and ongoing progress. On January 1, 2011, she became the Director of Nursing and continued as the MDS Coordinator/Restorative Coordinator. Recently the facility hired a full time MDS Coordinator. allowing Patty to focus soley on the Director of Nursing position. She resides in Centerville with her husband Kendell. They have four boys and two granddaughters who she loves spending time with. She also enjoys gardening.
Lori Hecht, Business Office Manager
Lori has been employed at Centerville Care and Rehab since November 2014. She had 12 years of experience as a Business Office Manager before joining our facility. Lori and her husband Kevin, live in Centerville. They have a son, two daughters, a daughter-in-law, a son-in-law, and two granddaughters. Another granddaughter is expected in February 2018. Lori loves being a grandma. Her favorite pastime is spending time with family and friends.

Jan Hult, Activities Director
Jan has worked with the facility since 1996. Jan has been an active member of the Centerville community for many years. Jan and her husband, Jerry, farm north of Centerville. Jan has always had a passion for long term care and providing quality of life for residents. Jan enjoys spending her free time with her 4 children and several grandchildren!
Jana Goetz, Environmental Services
Jana has been working at the facility since December 2003. She has worked in a variety of departments, such as Housekeeping, Dietary, Nursing, and Maintenance. She is now the supervisor of Housekeeping, Laundry, and Maintenance. She currently lives in Centerville with her husband, Chris, and two children.
Doug Bryenldson, Dietary Manager
Doug began working as a cook at Centerville Care and Rehab Center in May 2008 and has been the facility's Dietary Manager since 2015. He has worked in various nursing home kitchens since 1985. Doug resides in rural Centerville and is intensely proud of his daughter.
