Formerly the Centerville Good Samaritan Center, Centerville Care and Rehab has been serving the community of Centerville and surrounding areas since first opening doors in 1961. In the past 50 years, we have kept our home town, family atmostphere while continuing to improve our quality of care by taking advantage of advancements in rehabilitation services, medical technology and employee education.

Patricia Knutson, Director of Nursing
Patty has been employed at the facility since September 9, 1997. At initial employment, she worked as a day shift charge nurse until 2007. At that time, she became the MDS Coordinator and monitored both inpatient and outpatient scheduling and ongoing progress. On January 1st, 2011, she became the Director of Nursing and continuted as the MDS Coordinator/Restorative Coordinator. She resides in Centerville with her husband Kendell. They have four boys and two granddaughters who she loves spending time with. She also enjoys gardening. Further plans are to continue at Centerville Care and Rehab Center, while building strong connections with the community and continuing to love for the residents and families.
Lori Hecht, Business Office Manager
Lori has been employed at Centerville Care and Rehab since November 2014. She had 12 years of experience as a Business Office Manager before joining our facility. Lori and her husband, Kevin live in Centerville. They have a son, daughter-in-law, and two daughters. She enjoys spending time with her family and friends.

Jana Goetz Environmental Services
Jana has been working at the facility since December 2003. She has worked in a variety of departments, such as Housekeeping, Dietary, Nursing, and Maintenance. She currently lives in Centerville with her husband, Chris, and two children.
Kristen Birgen, Dietary Manager
Kristen began working at Centerville Care and Rehab Center in August 1997 while in high school. She was hired as a dishwasher and cook. She began the dietary manager course in 2002 and later became the dietary manager in 2004. Kristen resides in Centerville and is married with two children.
Bert Surprenant, Laundry Director
Bert has been working at the facility for seventeen and half years. Most of the time being spent in the laundry department. Bert has been married to Terry for thirty-four years and have two children with one grandchild. They have lived in the Centerville area the entire time.
