About Us

Formerly the Centerville Good Samaritan Center, Centerville Care and Rehab has been serving the community of Centerville and surrounding areas since first opening doors in 1961. In the past 50 years, we have kept our home town, family atmosphere while continuing to improve our quality of care by taking advantage of advancements in rehabilitation services, medical technology and employee education.

Team

Sam Van Voorst, Administrator
Sam graduated from USD obtaining his Health Care Administration degree.  He has been working with our sister facility, Dells Nursing & Rehab Center since 2012 and has been the Administrator at Centerville Care & Rehab since 2017.   In 2019 him and his wife, Marysa, purchased their first home in Centerville, SD.  Sam is on the Centerville Ambulance board, Rotary, Chamber of Commerce.  In his spare time he likes to go golfing and fishing. 

Patricia Knutson, Director of Nursing
Patty has been employed at the facility since September 1997. At initial employment, she worked as a day shift charge nurse until 2007. At that time, she became the MDS Coordinator and monitored both inpatient and outpatient scheduling and ongoing progress. On January 1, 2011, she became the Director of Nursing and continued as the MDS Coordinator/Restorative Coordinator. Recently the facility hired a full time MDS Coordinator allowing Patty to focus solely on the Director of Nursing position. She resides in Centerville with her husband Kendell. They have four boys and four granddaughters who she loves spending time with. She also enjoys gardening.

Lori Hecht, Business Office Manager
Lori has been employed at Centerville Care and Rehab since November 2014. She worked at two other long-term care facilities prior to joining our team. Lori grew up in Centerville. She and her family moved back to town in 2007. Lori and her husband, Kevin, enjoy camping, bike riding, playing cards and board games. Her favorite past-time is spending time with her family. She especially enjoys time spent with her two beautiful granddaughters.

Kristen Birgen, Social Services

Kristen has been employed at the facility since August 1997.  She started out as a dietary assistant/cook and became Dietary Manager in the Fall of 2004.  In 2015 she went to PRN status pursuing a career in daycare and returned to the facility full-time

in Spring of 2018 as SSD.  Kristen strives to educate herself on Parkinson’s Disease and the elderly.  Kristen resides in Centerville with her husband Greg and two children.  When she is not working she enjoys spending time with family and traveling.

Breanne Nelsen, Activities Director
Breanne has worked for the facility since June 2018.  Breanne has been working in independent and skilled care since she was 16 years old.  She has a strong passion for the elderly and continues to educate herself on Dementia/Alzheimer’s.  In 2018 her and her husband, Justin, purchased an acreage near Centerville.  In 2019 they welcomed their baby girl, Jaycee, into this world.   Breanne enjoys many activities outdoors and spending time with her family. 

Ron Carlson, Environmental Services
Ron has been employed at Centerville Care and Rehab since August 2019.  Ron was born and raised in the Centerville area.  While in the Army he met and moved with his wife, Ann.  They have three sons.  Upon returning from the Army he attended Junior College and attained his Associates of Arts degree.  He is happy to have the opportunity to work in his home community.

Teresa Jensen, Dietary Manager
Teresa has been employed at Centerville Care and Rehab since October 2015.  She has previously worked at Sanford Hospital in dietary.  Teresa grew up in Centerville and currently resides here with her husband, Brian Jensen.  Teresa would love to admit she likes to garden but that is solely on her husband, Brian.  Together, they enjoy spending time riding their Harley Davidson motorcycle and spending time with their 7 grandchildren.
